Registration
You may register for the 2nd International Congress on Early Onset Scoliosis and Growing Spine by completing and mailing or faxing the registration form.
You may also register online at www.regonline.com/ICEOS2008.
All registrations will be confirmed via e-mail. If you do not receive a confirmation within two weeks of submission, please contact the ICEOS Meeting Planning Office at (630) 681-1040 ext. 311.
Register early as the limited space will fill rapidly. Surgeons and corporate attendees who register after the course is full will be placed on a wait-list and contacted if space becomes available.
Registration Fees
On or before September 17, 2008

The registration fee includes entrance to the scientific sessions, course refreshments, lunches, reception, and course materials. Corporate attendees will be seated in the general session only after all medical attendees are accommodated.
The registration cut-off date is October 20, 2008. Registrations will only be accepted on-site after this date.
Cancellations & Refunds
All cancellations must be received in writing. The ICEOS Meeting Planning Office will grant a full refund for cancellations received in writing on or before September 17, 2008. For written cancellations received after September 17 a $150 processing fee will be withheld from your refund. NO REFUNDS will be granted after October 20th.
Attendance
The course chairmen reserve the right to refuse admission to anyone whose presence is considered to be incompatible with the course objectives.
